Policies and Procedures

Payment: Evaluation fees are to be collected before the final parent meeting. An invoice will be sent to the client/parent/guardian’s email. A card must be placed on file for recurring payments. The credit card will never be run unless services have been provided or in the case of a no-show (addressed below). Sessions cannot continue unless the previous session has been paid in full.

Cancel for Illness: If you/your child is sick or unable to attend therapy, please contact your therapist immediately to cancel and reschedule the session. Please do not bring your child to the office if he/she is sick. We ask that you/your child be well for 24 hours, including free from fever and/or vomit/diarrhea, as well as on any necessary antibiotics for 24 hours. If you or another family member is sick, resulting in a missed session, it is encouraged to switch to a teletherapy platform for the session if possible.

General Cancellation: A notice of at least 24 hours is necessary for all cancellations.

Late Cancel (Less than 24 hours notice): A notice of at least 24 hours is necessary for all cancellations. If you need to cancel a same-day session, please contact your child’s therapist immediately. If you cancel with less than 24-hour notice, a cost of half the session rate will be charged. Any 3 consecutive late cancels will result in being removed from the schedule.

No-Show: A notice of at least 24 hours is necessary for all cancellations. If you fail to attend a session without notifying your child’s therapist, the cost of the session will still be charged. You will be contacted regarding your session. Lack of communication following a no-show will result in being removed from the schedule until communication is made. Any 3 consecutive no-shows will also result in either being removed from the schedule or being moved to a different time.

Tardiness: If you arrive late to your session, the cost of the session does not change. Your child will be seen for therapy in the time remaining in the session. If your child is not present for at least 1/2 of the session, the session is to be treated as a no-show. See above.

Late Pick-up: The length of your child’s session is 45 minutes. In order to ensure there is time for clean-up and adequate time for preparation for other sessions, it is important to pick up your child at the

designated time. In cases of late pickup, a charge of $15 for every 15 minutes will be added to your therapy costs. If you are running late for any reason, please reach out to your therapist as soon as possible. Your therapist understands that unavoidable situations do arise and can be accommodated for in certain circumstances.

Teletherapy: If an in-person therapy session is not possible for a session or multiple sessions, use of a secure teletherapy platform is highly encouraged to avoid a disruption in therapy services for your child. If teletherapy is not appropriate for your child or is not an effective form of treatment for your child, then this can be disregarded and therapy services can be put on hold and rescheduled when able to resume in-person sessions.

Snow Days: Snow days are dependent upon the comfort level of both the client and therapist driving in given conditions. Cancellations as a result of inclement weather do not result in being charged. At times, virtual sessions during inclement weather can be an option as well.

Out-of-clinic Sessions: Out-of-clinic sessions, including lifestyle sessions, in-person school meetings, school sessions, and school observations are subject to the therapist’s availability concerning the date and time of the session. All suggested locations must be within a reasonable distance from Bloom Brightly Therapy, LC. Your therapist will make every effort to be as flexible as possible with these sessions.

Discontinuation of Services: The therapist is able to discontinue services for any client at any time if deemed necessary by the therapist. Likewise, the child’s guardian is able to choose to discontinue services at any time. Refunds are not an option for any services that have already been provided.

Waitlist: If the therapist is not currently accepting new clients, you can elect to have your child placed on a waitlist. At this time, you will be asked to sign the fee agreement in order to ensure the cost of services reflect when you first sought out therapy services, rather than when you receive services in case prices were to be increased before your child is able to come off the waitlist and receive therapy. If you refuse services when contacted to receive therapy services, the fee agreement will be void and your child will be removed from the waitlist. Should you decide at a later time to pursue therapy services with Bloom Brightly Therapy, the process will be the same as above.

Custody Litigation: The therapist's policy is not to testify in custody disputes. The therapist does not allow treatment records to be read or reviewed by any individuals. If you are involved in a divorce or

custody litigation, the occupational therapist is not to make recommendations to the court concerning custody or parenting. By signing this statement, you agree to not subpoena me to court for testimony or for disclosure of treatment information in such litigation; and you agree not to request that I write any reports to the court or to your attorney, making recommendations regarding custody. The court can appoint professionals, who have no prior relationship with family members, to conduct an investigation or evaluation and make recommendations to the court concerning parental responsibilities or parenting time in the best interest of the child(ren) involved. If Bloom Brightly Therapy does become involved in a legal matter out of requirement, the starting fee is $5,000 payable in advance. If you provide a release for you or your child’s records to an attorney, this is considered involving me in your court/legal matter.
